Sliding Fee Discount Program
We understand that behavioral health care can be a financial investment. For eligible self-pay patients, we offer a Sliding Fee Discount Program that may help reduce the cost of certain services based on household income and household size.
Eligibility is determined individually by our billing team after review of the required information and documentation.
Services That May Qualify
Our Sliding Fee Discount Program may apply to eligible self-pay services, including:
- Medication management appointments
- Therapy initial assessments
- Established patient therapy visits
Some services may not qualify.
How It Works
1. Contact Our Billing Team
Speak with our billing team to request a sliding fee screening.
2. Complete the Required Application
Our billing team will provide the application and explain what is needed.
3. Submit Income Documentation
You may be asked to provide proof of household income, such as recent pay stubs, tax documents, or benefit statements.
4. Eligibility Review
Our billing team will review your household size, income, and documentation.
5. Receive Your Approved Rate
If approved, you will be notified of your discounted self-pay rate before services are billed at that rate.
Documents You May Need
Depending on your situation, you may be asked to provide:
- Government-issued photo ID
- Proof of household income
- Three most recent pay stubs
- Prior year tax return
- Social Security, disability, or unemployment benefit statement
- Other income documentation, if requested
Important Information
Sliding fee discounts are available only for eligible self-pay services. Payment is due at the time of service unless other arrangements have been approved.
Submission of an application does not guarantee approval. Discounts cannot be applied until eligibility has been reviewed and approved by our billing team.
Approved discounts may need to be renewed periodically or sooner if household income, household size, or insurance coverage changes.
